Resumo: Subgemmal neurogenous plaque is a regional anatomic variation characterized by ganglion cells subepithelial nerve plexus close to the taste buds of the tongue. A 40-year-old female patient was referred for evaluation of a burning sensation on the right lateral border of her tongue which had started 9 months ago. Intraoral examination revealed a erythematous area and hyperplastic foliate papillae on the left lateral border. The diagnostic hypotheses were foliate papilitis and subgemmal neurogenic plaque. The lesion was surgically removed and histopathological examination followed by immunohistochemical staining for S100 were performed. Microscopically, a superficial layer revealed a neurofibroma-like proliferation parallel to the surface epithelium, and a deep area composed of tortuous neuronal bundles with ganglion cells, as in a neuroma-like lesion compatible with the diagnosis of subgemmal neurogenic plaque. The patient was asymptomatic at a 9-month follow-up.
