Título: Maxillary plasmacytoma as an early sign of multiple myeloma

Resumo: Plasmacytoma is a rare neoplastic proliferation of plasma cells that can involve soft tissue
(extramedullary) or bone. It can be associated with multiple myeloma (MM) or occur as a
solitary lesion. Sometimes it's the first sign of MM or its recurrence. A 42-year-old female
patient developed a painless fibroelastic nodule (5 cm) with telangiectasias in the left side of
the maxilla, the color was similar to the adjacent mucosa. Imaging exams showed a
unilocular radiolucent lesion between teeth 27 and 28. Incisional biopsy showed diffuse
infiltration of plasma with positivity for CD138 and lambda light chain, and negativity for
CD56 and kappa light chain. Additional laboratory exams were consistent with MM.
Chemotherapy treatment was initiated with the bortezomib, thalidomide, and
dexamethasone protocol, with partial regression of the oral lesion. At this moment, the
patient is being followed-up and is scheduled for autologous hematopoietic stem cell
transplantation.
