Título: Maxillary plasmacytoma as an early sign of multiple myeloma

Resumo: Plasmacytoma is a rare neoplastic proliferation of plasma cells that can involve soft tissue(extramedullary) or bone. It can be associated with multiple myeloma (MM) or occur as asolitary lesion. Sometimes it's the first sign of MM or its recurrence. A 42-year-old femalepatient developed a painless fibroelastic nodule (5 cm) with telangiectasias in the left side ofthe maxilla, the color was similar to the adjacent mucosa. Imaging exams showed aunilocular radiolucent lesion between teeth 27 and 28. Incisional biopsy showed diffuseinfiltration of plasma with positivity for CD138 and lambda light chain, and negativity forCD56 and kappa light chain. Additional laboratory exams were consistent with MM.Chemotherapy treatment was initiated with the bortezomib, thalidomide, anddexamethasone protocol, with partial regression of the oral lesion. At this moment, thepatient is being followed-up and is scheduled for autologous hematopoietic stem celltransplantation.
