Resumo: A 19-year-old female was referred to the stomatology clinic complaining of a "swelling in the upper lip". Physical examination revealed a submucosal fibrous movable nodular lesion measuring 3.5 cm in the upper lip. Medical history revealed mental illness and episodes of epileptic seizures. Clinical diagnosis was pleomorphic adenoma, and an excisional biopsy was performed under general anesthesia. Histological analysis of the HE stained slides revealed a proliferation of large polygonal cells with abundant eosinophilic granular cytoplasm and hyperchromatic pleomorphic nuclei with prominent nucleoli within the connective tissue. Immunoreactions excluded other mesenchymal tumors and positivity to S-100, alpha-inhibin, SOX-10, together with ki-67 expression in 7% of the tumor cells, supported the diagnosis of atypical granular cell tumor. The patient was referred to a plastic surgeon and is now under clinical follow-up.
