Resumo: Objective: To investigate the accuracy of autofluorescence in the diagnosis of OPDMs by correlation between clinical features and low- and high-risk oral epithelial dysplasia (OED) grades. Methods: Conventional oral examination (COE) and autofluorescence were performed in 16 patients with suspected OPMD. By COE, recorded clinical features of the lesion. By autofluorescence, the fluorescence visualization loss (FVL) and fluorescence visualization retained (FVR). The included subjects underwent biopsy. Lesions were divided: OPMDs with moderate or severe OED - high risk and OPMDs with mild or absent OED - low risk. Results: Three lesions were classified as high risk and 13 as low risk. Six lesions demonstrated FVL and 10, FVR. Correlating histopathology with COE and autofluorescence, 2 lesions were true positive, 1 false negative, 5 false positive and 8 true negative with mostly FVR and mild OED or absence - low risk. Conclusion: Autofluorescence alone does not predict the risk of malignancy compared to histopathological examination. The accuracy is limited as a small sample, however, because of the true negative results, this diagnostic method may contribute to the screening of lesions and the selection of areas for biopsy in the management of oral potentially malignant disorders.
