Resumo: A 20‑year‑old male presented with an asymptomatic left midfacial swelling. On intraoral evaluation, a volume was appreciable in the left labial mandibular with dental mobility of vital elements 32 to 36. The radiographic findings showing a well-defined unilocular radiolucency, measuring 4 cm in size, with root resorption of dental elements. Radiological examination associated with aspiration positive of cystic fluid suggested odontogenic keratocyst, unicystic ameloblastoma and calcifying odontogenic cyst (COC) as provisional diagnoses. Management initially consisted in the incisional biopsy with decompression of the lesion. Histopathology showed a cystic wall bordered by an odontogenic epithelium whose basal layer was made of ameloblastic differentiation and ghost cells, confirming the diagnosis of COC. The patient underwent complete enucleation of the lesion, and no recurrence was noticed after an 8-months follow-up. COC is an unusual developmental odontogenic cyst that simulates other more common jaw entities. The follow-up is advocated to prevent recurrence.
