Resumo: Objective: this study investigated early and late clinical and histological changes caused by the application of calcium hydroxyapatite (CaHA) in the orofacial region. Study Design: 48 Wistar rats were distributed into control and experimental groups. The CaHA was applied in the ventral surface of the tongue and submandibular region and the animals were euthanized after seven, 30 and 90 days. Results: the formation of nodules on the tongue was observed after seven days. There was migration of the material to the base of the tongue in two animals. Histopathological examination revealed CaHA spheres surrounded by an abundant infiltrate, composed predominantly of macrophages. In the experimental group, the percentage of collagen was higher compared to the control group (p<0.05) at 30 and 90 days. The thickness of the epidermis/dermis was also higher in the CaHA group (p<0.05). In the submandibular glands, which contained the material, edema, hyperemia, and inflammatory infiltrate were observed. Morphological alterations in the ducts and acini were also evidenced. Conclusion: this study demonstrated the volumizing capacity of CaHA, and its ability in inducing inflammatory response, increase in collagen density and thickness of the epidermis/dermis. More studies are needed to investigate the possibility of migration and the glandular changes.
