Resumo: A 89-years-old female patient was admitted to our center for evaluation of a painful ulcer in palatine mucosa. She reported an evolution time of 20-days, since then she traumatize the region with a hairpin in an attempt to relieve the pain caused by the ulcer. The intraoral examination revealed a painful ulcer in palatine mucosa, adjacent to teeth 25, 26 and 27, measuring approximated 2.0x1.0 cm, with well-defined limits, and slightly raised edges. Based on a diagnostic hypothesis of inflammatory ulcer, granulomatous infection or malignant neoplasm, a biopsy was performed. Histopathological examination revealed an ulcerated lesion, with fibroleukocyte crust, presence of moderate superficial and deep inflammatory infiltrate with eosinophils, permeating proliferative vessels. The final diagnosis was eosinophilic ulcer of oral mucosa. A partial healing process was observed three weeks after the surgical procedure. This case demonstrates that eosinophilic ulcer should be considered in the differential diagnosis of extensive gingival lesions.
