Resumo: A 57-year-old woman was referred to our service for evaluation of a nodular lesion, with spontaneous bleeding, in oral mucosa. According to the patient, the lesion had five years of evolution. In the last year, she reported several episodes of intense bleeding and difficulty in controlling hemorrhage. In her medical history, the patient also showed episodes of epistaxis and spontaneous skin hematomas. Extraoral examination revealed some petechiae on the face. Intraoral examination displayed a submucosal nodule, purplish, measuring about 1.5 x 1.0 cm in upper lip mucosa. After clinical evaluation, hematological tests were requested. Red and white blood cell count were normal. However, the low platelet count revealed a severe thrombocytopenia. A low prothrombin activity was also detected. The patient was then referred to hematologist, which indicated idiopathic thrombocytopenic purpura. After the diagnosis, the patient started the hematological treatment and is under clinical follow-up with our interdisciplinary team.
