Resumo: Desmoplastic ameloblastoma (DA) is a rare odontogenic tumor of epithelial origin that is distinguished from classic ameloblastoma in terms of anatomical distribution and clinical presentation. This lesion has an incidence that varies from 4 to 13% among other ameloblastoma variants. Here, we reported a case of a 51-year-old female patient, non-smoker and non-alcoholic who underwent evaluation of a bone defect on the right anterior mandible. Imaging exams showed a mixed and multilocular lesion. Histopathological examination revealed small islands and cords of odontogenic epithelium surrounded by desmoplastic reaction. According to these findings, the diagnosis of desmoplastic ameloblastoma was established. Desmoplastic ameloblastoma is a possible differential diagnosis for lesions exhibiting radiographically a mixed radiopaque-radiolucent aspect with ill-defined borders. DA is a variation among ameloblastomas with clinicoradiographic and histopathologic distinct features. The correct diagnosis of this variant contributes to clarifying the clinical and biological behavior of desmoplastic ameloblastoma, which is still unknown.
