Resumo: Carcinoma Ex Pleomorphic Adenoma (CXPA) is a rare salivary gland neoplasm, typically resulting from a pre-existing pleomorphic adenoma (PA). We present the case of a 69-year-old male patient who was referred for evaluation of a hard palate lesion with more than 40 years of duration and a previous diagnosis of PA. Intraoral examination revealed a circumscribed asymptomatic nodule in the hard palate region. An incisional biopsy was performed. Histopathological examination revealed a malignant neoplasm of glandular origin characterized by the proliferation of myoepithelial and ductal cells, with ovoid and sometimes fusiform or plasmacytoid morphology. An area with histological benign components of PA and hyalinization was evident tt the periphery of the lesion. Immunohistochemical analysis showed positivity for CK7, S100, VIM and p63. and negativity for AML. Despite its low incidence and uncertain pathogenesis, CXPA must be included in the differential diagnosis of neoplasms of glandular origin.
