Resumo: The radicular cyst is the most common inflammatory odontogenic cyst, typically occurring in the periapical region as a result of endodontic infection. However, it can also occur laterally to the root, mimicking the lateral periodontal cyst. We present a case of a 75-year-old white man with pain in an endodontically treated tooth (44), with coronal destruction and edematous gum. Radiographically, a circular radiolucent area surrounded by a radiopaque halo was observed at the mesial cervical third. Surgical removal of the tooth with the cystic lesion adhered laterally to the root was performed, and histopathological analysis revealed a virtual cystic cavity lined by stratified squamous epithelium, with fibrous connective tissue and mononuclear inflammatory infiltrate in the cystic capsule. The diagnosis was radicular cyst. Diagnosis of radicular cyst occurring laterally can be challenging to the clinical due radiological similarity with lateral periodontal cyst, requiring clinical and histopathological correlation for accuracy diagnosis.
