Resumo: The aim was to describe a case of a patient referred to a specialized service to diagnose lesions in the oral mucosa, lips, and a generalized skin manifestation. A 69-year-old melanoderma female patient with a history of high blood pressure and chronic kidney disease complained of lesions that initially developed on the arms, spreading to the body and mouth, and associated with painful symptoms. Hyperpigmented patches on the neck, dorsum, arms, and legs were identified. White striae in the nails were also present. Intraoral examination revealed whitish and striated plaques on the bilateral buccal mucosa and lips, with some focal areas of ulceration. Incisional biopsy revealed a lichenoid infiltrate. The final diagnosis of lichen planus pigmentosus was made and dexamethasone elixir was prescribed. After 15 days, a partial remission of the symptoms and significant improvement were observed. The patient is still on follow-up with both rheumatology and stomatology services.
