Resumo: This study aims to describe a case series of oral Plasmablastic lymphoma (PBL), a high-grade lymphoma that affects the oral cavity, with a strong association with immunodeficiency and EBV infection. Twenty-two cases were retrospectively obtained and characterized based on clinical, microscopic, immunohistochemical and molecular profile. Patients mean age of 45.71 years (range 23-81) and male predilection was observed. Lesions presented as an ill-defined swelling with ulcerated surface, affecting mainly the gingiva and palate.Tumors revealed a diffuse proliferation of large cells resembling immunoblasts and/or plasmablasts, and the presence of mitotic figures, apoptotic cells and tingible body macrophages.Immunohistochemistry was positive for CD138, VS38c, MUM1, negative for CD20, and high proliferative index (>90%). In situ hybridization showed the presence of EBV in all cases, while MYC translocation was seen in 11 (57.9%) cases. Treatment of choice included chemotherapy associated with highly active antiretroviral therapy (HAART).PBL is an aggressive neoplasm with a poor prognosis.
