Resumo: A 40-year-old female patient presented with peripheral facial paralysis on the right side, diplopia, followed by cervical, facial, and auricular pain in the right hemiarch, and headache. Laboratory tests did not show reactivity for Herpes Simplex type I and II, and Varicella Zoster, with negative IgM and positive IgG. The treatment used was Acyclovir (1000 mg five times daily, for 21 days.), Prednisone (15 mg/d for days to 21), Tramadol (100 mg once a day, as needed), and low level laser therapy (285J/cm2, 100mW, 808nm two times weekly), which led to partial improvement of peripheral facial paralysis associated with pain reduction. The patient is being followed up and receiving multidisciplinary care. This case emphasizes the importance of early diagnosis and timely treatment for a good prognosis, avoiding the possible presence of mutilating motor nerve sequelae.
