Título: ODONTOGENIC FIBROMYXOMA MIMICKING LOW-GRADE FIBROMYXOID SARCOMA: CASE REPORT

Resumo: Low-grade fibromyxoid sarcoma (LGFS) has been reported in the oral and maxillofacial region. Sometimes, as previously reported, odontogenic (fibro)myxoma can mimic LGFS, on microscopy. A 29-year-old female was referred complaining discomfort during chewing 1 month ago. Intraoral examination showed a painless tumoral mass in the posterior region of the maxilla causing tooth displacement. Panoramic radiography revealed an irregular radiolucency and CT scans evidenced a hypodense area, promoting bone cortical expansion with maxillary sinus involvement. After incisional biopsy, microscopy showed ample areas containing spindled to stellate cells with eosinophilic cytoplasm, perivascular hyalinization, admixed with irregular myxoid areas. Moreover, scarce foci of odontogenic epithelium were observed. Immunohistochemical analysis showed positivity for CK AE1/3 and p63 in the epithelial component, while S100, SMA, desmin and, relevantly, MUC4 were negative. Rare cells were Ki-67 positive. The diagnosis was odontogenic fibromyxoma with LGFS features. After 3-year of follow-up, there are no recurrences or alterations.
