Resumo: A 65-year-old woman complaining of gingival lesion was attended in our clinic. She was using Malvona® mouthwash which caused a burning sensation. Intra-oral examination showed a brownish lesion with a smooth surface and fibrous consistency in upper posterior vestibular and palatine gingiva. Moreover, ulcers were presented in the buccal mucosa. Mouthwash was discontinued and chlorhexidine 0,12% twice a day was prescribed. In the next patient's visit, a brownish lesion was maintained but without ulcer. Our main diagnosis hypothesis was melanoma and incisional biopsies were performed on both vestibular and palatine gingiva. Histopathological and immunohistochemistry exams (MelanA, HMB45 and S100) were conclusive of melanoma. The patient was referred to treatment which consisted of surgery alone.Oral melanoma is an aggressive neoplasm extremely rare in the mouth and early diagnosis is very important for the prognosis. Oral pigmentations can be commonly neglected, and the biopsy must be performed in suspicious lesions.
