Resumo: The prevalence of oropharynx carcinoma, potentially HPV-related carcinoma, is increasing. A 42-years-old male patient, admitted to the infectious diseases hospital complaining of odynophagia, dysphagia and oral and oropharynx lesions, was referred to the stomatology service for evaluation of these lesions that was unsuccessful treated with multiple drugs. HIV infection since 2011, smoker and alcoholic for more than 30 years. Positive cervical and contralateral submandibular lymph nodes were observed. Multiple papillary lesions, a non-removable white plaque (1.5x1.0cm) on the soft palate and a tonsil's painfull tumor with verrucous surface (4.0x2.0x1.0cm). The clinical diagnosis were leukoplakia and Oral Squamous Cell Carcinoma (OSCC). Incisional biopsies were performed. The histopathological analysis showed moderate epithelial dysplasia and hyperkeratosis in soft palate and OSCC in the oropharynx. The patient was referred and is awaiting oncological treatment. This case illustrates the difficulty still found by the health team for the diagnosis of SCC in AIDS patient.
