Resumo: Syphilis is a sexually transmitted infection caused by the bacterium treponema pallidum. Different oral manifestations are described which may contribute to the diagnosis of the disease. A 57-year-old male presented with a moriform ulcerated lesion in the alveolar ridge measuring approximately 4 cm and lip edema. The diagnostic hypotheses were paracoccidioidomycosis, histoplasmosis and squamous cell carcinoma. An incisional biopsy revealed a fragment of mucosa covered by hyperplastic stratified squamous epithelium and in the lamina propria a vascularized fibrous connective tissue was noted, showing an intense mixed inflammatory infiltrate, predominantly lymphoplasmacytic and an abundance of perivascular plasmocytes. Vdrl and fta-abs tests confirmed the diagnosis of syphilis, and the patient underwent treatment with benzathine penicillin. After the use of medication and reassessment, total remission of oral lesions was observed. This case reinforces the importance of studying the oral manifestations of syphilis and its different oral manifestations
