Título: RISK FACTORS FOR MEDICATION-RELATED OSTEONECROSIS OF THE JAW IN ONCOLOGICAL PATIENTS

Resumo: There is lack of information regarding the risk factors for medication-related osteonecrosis of the jaw (MRONJ) in oncological patients. This retrospective observational study evaluated the clinical factors related to the risk for MRONJ occurrence in a cohort of cancer patients using zoledronic acid (ZA). Cancer patients with a history of ZA use between June 2016 and February 2019 were included. Demographic data, comorbidities, oncological and medication information, and dental history were collected. Risk analysis was performed using multivariate logistic regression adjusted for variables with significance in the exploratory analysis. 354 patients were included, the median age was 73 years (23-99), 72% were male, 67.8% underwent dental treatment as preparation for ZA use, and 5% developed MRONJ. The analysis showed as independent risk factors the longer time of ZA use (Odds: 2.20-CI:1.11-4.59), and the presence of nutritional, metabolic and endocrinological diseases (Odds: 5.16-CI:1.28-21.34). While the lowest frequency of drug use (Odds: 0.20-CI:0.05-0.71), and the dental treatment prior starting ZA (Odds: 0.05-CI:0.01-0.23) were protective factors for MRONJ development. This was the first Brazilian study looking for risk factors in oncological ZA users. It was also the first study to demonstrate the importance of dental treatment as MRONJ prevention.
