Resumo: Osteoporotic focal bone marrow defect, found in the maxillary bones consists of a condition characterized by a radiolucent, asymptomatic area, often observed in the posterior mandible of middle-aged adult women. Objective: Clinical case of focal osteoporotic bone marrow defect associated with fixed prosthesis that caused expansion of the lingual bone plate of the mandible. Study: A 56 years old female patient came to the dental office for evaluation of asymptomatic mandibular bone growth. Design: Incisional biopsy was performed and the material was forwarded for histopathological analysis. Results: Microscopic sections revealed bone marrow with presence of erythrocytes, leukocytes and megakaryocytes permeating adipose tissue and thin irregular bone trabeculae. Conclusion: The established diagnosis was focal osteoporotic bone marrow defect. This clinical case reinforces that, although osteoporotic bone marrow defect with bone expansion is uncommon, histopathological examination is indispensable for definitive diagnosis.
