Resumo: A 31-years-old woman, COVID-19 infection history, presented a white plaque on the right border of the tongue, measuring 1cm in diameter, 2 months of evolution, and light discomfort. The initial clinical diagnosis was lichen planus versus oral leukoplakia versus trauma. An incisional biopsy was performed, and the histopathological analysis showed the presence of microabscesses on the epithelium, an intense mononuclear inflammatory infiltrate with predominance of plasmocytes in the subepithelial and perivascular region. In some areas, the plasmocytes presented binucleated nuclei and cell atypia. Due to the plasmocyte alteration, the pathologists suggested different diagnoses for investigation: plasma cell mucositis, corticosteroids induced lesion after COVID-19 treatment, cytomegalovirus infection, and syphilis. The clinician was oriented to investigate the patient's syphilis history as a diagnosis of exclusion. After syphilis tested positive, the patient was referred to the doctor for antibiotic treatment. After two months of antibiotic treatment, the white plaque showed complete clinical regression.
