Título: DIFFERENTIAL DIAGNOSIS OF SWELLING IN THE FLOOR OF THE MOUTH: A CASE REPORT

Resumo: The diagnosis of non-classical lesions can be challenging. A 65-year-old female patient presented a swelling in the left floor of mouth, two-weeks of history. Pain and fever were reported, enlarged submandibular lymph nodes identified. Salivary flow was absent in the left parotid and submandibular glands. The 4cm firm swelling was covered by normal mucosa, extending from floor of mouth to the submandibular region. Differential diagnoses included sialadenitis and neoplasia. No significant radiographic findings. Magnetic resonance imaging revealed ill-defined signal in the topography of sublingual gland, with a medial cystic area, no lymph nodes enlargement, supporting the hypothesis of neoplasia. Histopathological analysis of incisional biopsy revealed mucous extravasation, minor salivary glands, and mild inflammatory infiltrate. Based on clinical and microscopic findings the final diagnosis was "ranula". The lesion regressed completely after biopsy, with no recurrence in four months. Systemic symptoms of infection and lymphadenopathy disappeared and urinary tract infection was detected.
