Resumo: Intraosseous lipomas are among the rarest of primary bone tumors and are very rarely seen in head and neck bones. They are usually asymptomatic and are detected incidentally on radiographs for other complaints. We present a case report of a 70-year-old female patient who visited the dentist with the intention of pre-prosthetic surgery to prepare a mouth to receive a new maxillary denture prosthesis. Intraoral examination revealed a swelling in the posterior maxilla on the left side with a hard consistency, covered by normal mucosa. The panoramic radiograph showed no alterations. During the pre-prosthetic surgery to regularize the alveolar ridge, an intraosseous lesion with characteristics of a lipoma was observed. An excisional biopsy was performed, and histopathological examination revealed mature adipocytes in a lobular architecture separated by delicate fibrous septa and the fibrous capsule. The definitive diagnosis was intraosseous lipoma. The patient remains under follow-up.
