Resumo: The accessory parotid gland (APG) is a salivary tissue with incidence being reported to be approximately 21% among human cadavers. Tumours originating from this structure are extremely rare, and constitute 7% of all parotid gland tumours. A 20-year-old female referred to a stomatology service complaining of increased volume for about 18 months. Clinical examination revealed a mobile nodule, palpable intraorally and extraorally, in the buccal mucosa, firm consistency, asymptomatic, well-defined contours and without associated lymphadenopathy. Ultrasonography showed a rounded hypoechoic image, without associated vascularization, in the angle region of the left mandible. An excisional biopsy was performed and during the intraoperative period, a nodular, encapsulated lesion was observed, whose location suggested origin from APG. Upon histopathological analysis, the conclusive diagnosis was Pleomorphic Adenoma. After complete excision with preservation of the capsule, the patient is undergoing stomatological follow-up with no clinical signs of lesion recurrence and a very satisfactory scarring appearance.
