Resumo: Graft-versus-Host Disease (GVHD) is an immune-mediated complication frequently seen in patients undergoing allogeneic hematopoietic stem cell transplantation (HSCT). Painful complaints, compromised diet, opportunistic infections and odynophagia are associated with the oral manifestation of GVHD. Male patient, 9 years old, diagnosed with severe combined immunodeficiency syndrome (SCID), submitted to HSCT at 8 years, evolved with lesions in the eyes, mouth and skin. Intraoral examination revealed painful ulcerated and erythematous lesions throughout the mouth, lichen planus-like lesions on the tongue, limitation in mouth opening. Diagnosis was GVHD. Drug therapy consisted of clobetasol propionate (0.05%), and sirolimus 1mg. Antimicrobial photodynamic therapy (methylene blue 0.01% for 5 minutes followed by irradiation: 660nm, 100mW, 4J/point), was performed twice a week. After 30 days a clinical improvement of the lesions was observed, as well as pain complaints, reaffirming the importance of the correct diagnosis and treatment of GVHD.
