Resumo: The ossifying fibroma (OF) is an uncommon benign bone lesion characterized by the proliferation of fibrous cells and the formation of mineralized tissue, most commonly found in young adults. The formation of aneurysmal bone cyst is an uncommon feature, which can make the diagnosis more challenging. We present a case of a 44-year-old woman with a hard mass involving teeth 35, 34, and 33, covered by normal-colored mucosa. Cone-beam computed tomography revealed a hypodense area with hyperdense foci surrounding the roots of the affected teeth, with cortical expansion. An incisional biopsy was performed, revealing trabeculae of mineralized tissue resembling bone and cementum-like foci, also, aneurysmal cystic spaces filled with blood and areas of hemorrhage, compatible with OF associated with aneurysmal bone cyst. The patient underwent excision of the lesion. OF with aneurysmal bone cyst is an uncommon lesion and it´s accurate diagnosis is achievable through clinical, radiographic, and histopathological correlation.
