Resumo: In 2017 a 56-year-old man was referred to dental clinic complaining of toothache in maxilla. In panoramic radiograph three well-defined circular radiolucent images in the right posterior region of the body and ramus of the mandible were incidentally revealed and detailed by cone-beam computed tomography. Due to the difficulty of surgical access, only two lesions were biopsied. The microscopic examination revealed features of odontogenic keratocyst. The final diagnosis associated with frontal bossing, palmar and plantar pits in addition to abnormalities in a chest X-ray, confirmed the diagnosis of Gorlin Goltz syndrome. The biopsied lesions had notable improvement with enucleation and marsupialization treatment. In 2023, after radiographic follow-up, we evaluated that the third lesion was more accessible and performed the same therapy. In this 7-year follow-up, the bone healing of the lesions was excellent and we remain alert to possible predispositions of the syndrome, mainly the potentially malignant ones.
