Resumo: The ectomesenchymal chondromyxoid tumor (ECT) is a rare and benign mesenchymal neoplasm that predominantly affects the tongue. A 15-year-old boy presented a normal-colored and smooth surface nodule, measuring 15mm on the lingual side of the left mandible, between 33 and 35 teeth. The lesion was asymptomatic, appeared approximately 6 months ago, and no history of local trauma was observed. A computed tomography (CT) revealed a circular isodense area with well-defined borders. The hypotheses of peripheral ameloblastoma and peripheral ossifying fibroma were raised. The lesion was excised, and microscopically exhibited a demarcated lesion organized in lobules, composed of round and spindle-shaped tumor cells, arranged in a myxoid and chondroid matrix. Immunohistochemistry showed diffuse positivity for GLUT1, S-100, CD56 and AE1/AE3. Molecular analyses revealed a RREB1-MKL2 fusion gene, confirming the diagnosis of ECT. The patient is free of recurrence two years after surgical removal.
