Resumo: OBJECTIVE: To show the physical limits of a patient after removal of the fibula. METHOD: The patient underwent resection of the body and ascending ramus of the mandible due to a bone lesion diagnosed as ameloblastoma. It was reconstructed with microvascularized fibular flaps (FMR). In the anterior region, a niche was built in the stump of the mandible, where the end of the fibula was introduced. It was fixed with titanium mini-plates. Postoperatively, the patient was released for physiotherapy and started walking and running, even participating in a 42km marathon 19months after surgery. RESULT: Female patient NMM, 39years old, presented ameloblastoma in the body and left ascending ramus of the mandible, which was resected. After tumor resection/immediate reconstruction was performed with FMR and returned to function, aesthetics. CONCLUSION: Full function recovery of the flap donor leg. It is necessary to encourage the patient to lead his normal life.
