Resumo: A 26-years-old woman presented with a slowly growing and asymptomatic lump in the right cheek, perceived one year before. Mouth opening enhanced the perception of the lesion as a solitary, well circumscribed and fibrous nodule near to the inferior limit of the zygomatic border. With a clinical diagnosis of lipoma, the lesion was surgically removed. Microscopy revealed multiple and juxtaposed cystic structures of variable sizes and few papillary areas, organized in non-encapsulated lobules. Cystic lining comprised single layered flattened or cuboidal (hobnail) cells (positive for CK7 and CK19, negative for CK14 and HMW CK), with sporadic evidence of apocrine secretion, surrounded by rare myoepithelial (vimentin positive) cells. Cysts were filled with basophilic secretion, with occasional calcified bodies. Scarce seromucous acini and leukocytic infiltrate were seen among polycystic lobules. These findings leaded the diagnosis of dysgenetic polycystic disease. There was recurrence after 18 months of treatment. Financial support: CAPES #001.
