Resumo: A 34-year-old brown skinned man went to the Oral Diagnosis Clinic complaining of an asymptomatic tumor on his gingiva that had been present for 18 months, which was affecting his speech and aesthetics. Intraoral examination revealed a well-circumscribed, pedunculated nodule in the attached gingiva of the anterior maxilla, covered by normal mucosa. An excisional biopsy was performed with the diagnostic hypothesis of a non-neoplastic inflammatory lesion. Histological analysis revealed a benign mesenchymal neoplasm of fusiform, oval, and stellate cells arranged in a fascicular pattern and permeated by a myxoid stroma. Immunohistochemical staining was positive for AML and negative for S100. Considering both analyses, the final diagnosis was an inflammatory myofibroblastic tumor (IMT). IMT is a soft tissue tumor that is frequently observed in the lungs and is rare in the oral cavity. It can present with variable clinical behavior, representing a challenge for diagnosis.
