Resumo: While the lateral border of the tongue is considered one of the preferred locations for squamous cell carcinoma (SCC), the dorsum of the tongue seems to be occasionally affected. We contribute with seven cases of SCC of the dorsal tongue diagnosed in a Brazilian Oral Pathology laboratory between 2016 and 2022. There were five men and two women with age ranging from 29 to 98 years old. The main clinical presentation was of asymptomatic fast-growing, reddish to whitish nodule (4 cases), and ulcer (3 cases) exclusively located in the dorsal tongue. Medical history of most patients included long-term smoke and drinking habits, previous esophageal cancer with cervical lymph nodes metastasis, erythematous lupus, and tuberculosis in HIV+ setting. In addition to oral cancer, clinical differential diagnoses also included reactive and infection lesions. Exclusive involvement of the dorsal tongue by SCC is uncommon and possibly related to immunosuppression status.
