Resumo: Bisphosphonates, used for osteoporosis and cancer treatment, have been associated with jaw necrosis as a complication. We report the case of a 49-year-old female patient, smoker with a history of Multiple Myeloma (MM) that presented with complain of bone exposure in the mandible and intense local pain with history of tooth extraction in the same region. She underwent autologous HSCT due to MM, and treatment with Zometa. Clinically, a bone exposure in the teeth 46 region was observed with intense pain. CT showed a lytic area in the right posterior mandible measuring 19 mm, with irregular thickening of the adjacent periosteum close to the mandibular canal. It was also possible to observe MM lytic lesions. Debridement of the necrotic bone was performed and covered with Bichat´s ball, under general anesthesia due to neuralgic pain. She is currently being followed up at the outpatient clinic, with closure of bone exposure.
