Resumo: Merkel cell carcinoma (MCC) is a primary cutaneous neuroendocrine carcinoma of aggressive behavior that usually appears as a rapidly growing violaceous nodule or plaque in the sun-damaged skin of elderly Caucasians. It is microscopically characterized by small to intermediate round blue cells with a salt-and-pepper chromatin pattern positive for neuroendocrine and epithelial markers. We herein contribute with a rare case of exuberant MCC of the face. An otherwise healthy 82-year-old woman presented with a 6 x 5.5 cm fast-growing, painful ulcerated nodule in the right cheek of unknown duration. Microscopically, there were infiltrative small round cells with pleomorphic nuclei and granular chromatin, which were positive for pan-cytokeratin and CK20 (paranuclear dot-like pattern), chromogranin and synaptophysin (cytoplasmic pattern), and for CD56 (membrane pattern). The Ki-67 index labeling was 80%. The final diagnosis was MCC. Clinicians should consider MCC during evaluation of a large-sized violaceous nodule of the face.
