Resumo: Oral lichen planus (OLP) is a chronic and inflammatory potentially malignant disorder with a malignancy rate ranging from 0 to 3.5%. This study aims to report a plaque/atrophic OLP case that evolved to squamous cell carcinoma 18 months after initial diagnosis. A 62-year-old white man presented white plaques on the tongue dorsum and symptomatic atrophic areas on the lingual apex diagnosed, after histopathological examination, as OLP. After nine months of follow-up, the lesions evolved into ulcerated areas that revealed moderate epithelial dysplasia in the second histopathological examination. Six months later, progression to a nodule with an ulcerated surface was observed. A new incisional biopsy was performed, and the histopathological examination was compatible with squamous cell carcinoma. The patient was referred for surgical oncological treatment, and radiotherapy was unnecessary due to the early diagnosis. The patient is under follow-up and currently has 14 months without signs of recurrence.
