Resumo: Head and neck dermoid cysts account for 7% of all dermoid cysts, with the most common locations being the midline neck, nose, nasolabial fold, and lateral third of the eyebrow. In the oral cavity, these cysts appear more frequently on the midline of the floor of the mouth. We present a 22-year-old male with an intraosseous lesion in the region of teeth 21, 22 and 23. Computed tomography scan revealed a well-defined hypodense lesion with palatine cortical resorption. The patient presented a history of trauma in tooth 21 that evolved in endodontic treatment. Exeresis was performed and after microscopic examination the lesion was diagnosed as a dermoid cyst. There was no sign of recurrence after 4 months of follow up. This case shows that dermoid cysts may be successfully diagnosed, even in an unusual location, and managed using a series of simple, but effective clinical procedures.
