Resumo: We describe a case that affects a 29 years-old male patient with an exuberant case of
plexiform ameloblastoma in the mandible. The patient complained of swelling,
paresthesia and limitation of mouth opening for 4 months. Extra oral examination
showed swelling in the ramus and body of the mandible in the right side. Intraorally,
the lesion presented as a painless, hardened, red-whitish in color with a
pseudomembrane. The lesion was about 6 centimeters in its largest diameter.
Computed tomography was requested and incisional biopsy was performed for
histopathological evaluation. The diagnosis of plexiform ameloblastoma was made. For
optimization of surgical time and predictability, virtual surgical planning was
performed. The patient underwent to segmented resection followed by primary
mandibular reconstruction with an autologous graft from a microvascularized fibula.
Currently, the patient has been followed up for 8 months, without any signs of
recurrence (clinical/imaging) with preserved masticatory and aesthetic functions.
