Resumo: Carcinoma ex pleomorphic adenoma (CXPA) is defined as an epithelial malignancy that arises from a pre-existing pleomorphic adenoma (PA). One of the most important prognostic factors associated with CXPA is the degree of capsular violation by the carcinoma, with lesions being subclassified as intracapsular (no capsular invasion), minimally invasive (1.5-6.0 mm of invasion) and frankly invasive (> 6.0 mm of invasion). However, this measurement depends on the assessment of the entirely resected tumor. A 77-year-old woman was referred for evaluation of an asymptomatic nodule with ulcerated surface in left palate. A computed tomography scan showed extension into the sinonasal region. An incisional biopsy was performed and its histopathology revealed an epithelial-myoepithelial carcinoma (EMC) intermingled with PA areas. Although the sample was an incisional biopsy, the diagnosis of frankly invasive EMC ex PA was settled by the clinical-radiological correlation. This appropriate diagnosis sets the grounds for adequate treatment planning.
