Resumo: Complex odontoma is a benign lesion characterized as an amorphous mass of mineralized tissue, commonly diagnosed by routine imaginological exams. This case reports a 33-year-old female patient, referred to the oral and maxillofacial surgery service due to an asymptomatic lesion in the mandible, diagnosed through routine panoramic radiography. During anamnesis, the patient reported being a former smoker, and clinical examination revealed an increase in bone volume in the right mandible only sensitive to palpation. The orthopantomography revealed a radiopaque area in the right mandibular ramus, measuring 2 cm. The hypothesis diagnosis of complex odontoma was confirmed through an excisional biopsy and anatomopathological analyses. The patient was lost to follow-up, and after 1 year returned to the service complaining of pain in the operated region. The orthopantomography revealed a residual lesion in the right mandibular ramus. Another surgical intervention was planned and the patient is still being followed up.
