Resumo: Central giant cell granuloma (CGCG) is a benign osteolytic lesion of the jaws of unknown etiology. The behavior, location and extension of the lesion added to the patient´s age will determine the choice of treatment. The aim of this study is to report the case of a 28-year-old patient with expansive CLCG in the left mandibular body and parasymphysis, causing facial asymmetry. Initially, the patient was treated with 6 applications of intralesional injection of triamcinolone (20mg/ml). Subsequently, after treatment failure, the patient underwent therapy with denosumab (120 mg) for 12 months. Five months after the end of therapy with denosumab, computed tomography (CT) showed complete calcification of the lesion. However, 6 months later, a new CT showed radiolucent intraosseous images suggestive of lesion recurrence. We opted for surgical treatment with curettage of the lesion. The patient has been followed up for 5 months without recurrence.
