Resumo: This study reports a case of a 36-year-old female patient who complained about itching and painful lesions that "walked through the mouth". The condition had a cyclical behavior, with remission and relapses of the lesions during seven months. Clinical examination showed swelling and erythema of the lip and buccal mucosa, with serpiginous tracks. After proper evaluation, the patient was diagnosed with oral larva migrans, and the lesions resolved after ivermectin administration. At 18-month follow-up, no sign of recurrence was observed. Larva migrans is a parasitic infection that rarely affects oral mucosa, representing a pitfall in oral diagnosis and a stressful condition for the patient. Oral health care providers should be aware of that and keep this disease in mind as a possible differential diagnosis for erythematous oral lesions.
