Resumo: A 46-year-old man reported persistent unproductive cough, weight loss, and pain in the buccal mucosa for 1 year. During the anamnesis, he reported a history of tuberculosis with treatment concluded 2 months previously. Extraoral physical examination revealed multiple deep ulcers on the forehead, forearm, and leg. Intraoral examination revealed multiple moriform ulcers throughout the oral cavity. Under the clinical diagnosis of paracoccidioidomycosis, an incisional biopsy was performed. Microscopically, there were well-formed granulomas with multinucleated giant cells, lymphocytes and multiple budding yeast forms with birefringent walls (PAS and Grocott-Gomori positive), The oral paracoccidioidomycosis diagnosis was established. The patient has been under Itraconazole therapy for 1 month and the oral injuries are in remission. This case report was discussed with an infectious disease physician who attested that tuberculosis was confirmed by immunochromatographic and sensitivity testing to identify the "Mycobacterium tuberculosis".
