Resumo: A 27-year-old male patient reported swelling in the anterior maxilla, without painful symptoms on palpation, which appeared about 4 years ago. In the anamnesis, the patient reported not having any general disorder. Clinically, there was a volume increase in the anterior region of the maxilla, causing facial asymmetry on the left side of the face. The cone beam computed tomography (CBCT) showed a hypodense, unilocular lesion related to the impacted canine and hyperdense material associated. Based on these characteristics, the clinical approach was an excisional biopsy and anatomopathological analysis. The microscopic sections revealed fibrous connective tissue of capsular appearance, partially covered by ameloblastomatous epithelium associated with numerous ghost cells and mineralized areas, permeated by areas of irregular calcification of varying sizes with islets and cords of odontogenic epithelium. The diagnosis was calcifying odontogenic cyst. The patient presented a good evolution, without complications, and has been followed up ever since.
