Focal myositis (FM) is a benign inflammatory process that affects a single muscle, rarely the tongue. Microscopically, FM presents myopathic changes, mixed inflammatory cell infiltrate, interstitial fibrosis, fissures in skeletal muscle fibers, and myofiber diameter variation. We describe five additional cases of FM affecting the tongue. Three were male and two female (mean age 56 years). Three patients had symptomatic short-term swelling, and each presented ulceration and submucosal nodule. Two patients reported association with COVID-19 infection and vaccine. Microscopy was consistent with FM. Immunohistochemically, M2 macrophages and T cells (CD4+ higher than CD8+ cells) were predominant. Focally, CD15+ granulocytes and CD20+ B cells were visualized. Polyclonal CD138+ plasma cells with predominance of IgG+ cells (rarely IgG4+) were detected. All patients evolved with complete resolution after biopsy. FM should be considered in the differential diagnosis of nodular lesions of the tongue, with a subset probably associated with COVID-19 infection or vaccine.
