Resumo: Lymphangioma is a rare vascular anomaly that affects the lymphatic vessels which a marked predilection for the head and neck region. There is no consensus about the most effective treatment of it. We report a case of a 15-year-old boy with macroglossia. Intraoral examination revealed a swelling on the dorsum of the tongue, approximately 4 cm in size, with numerous small transparent vesicles like "frog-eggs" and diagnostic hypothesis of lymphangioma. The excisional biopsy was performed with a high-power diode laser. Histopathological revealed large, dilated spaces lined by endothelial cells liked lymph vessels close to the surface epithelium, confirming to be a lymphangioma. The patient is under follow-up six months, with no clinical signs of recurrence. High-power diode laser ablation was a safe and effective tool for the biopsy and treatment of this lesion, which provided the patient with a faster, efficient transoperative and a more comfortable postoperative period.
