Resumo: Paracoccidioidomycosis is a fungal infection more frequently observed in Latin America, being a relevant public health problem in Brazil. This paper reports a clinical case of a female, 60 year old patient, who reported severe pain in the oral region, lasting two years. Ulcerated lesions with a moriform appearance extending mainly to the belly, dorsum and lateral hedges of the tongue were observed during the physical examination. Due to the fact that the patient presented decompensated systemic arterial hypertension, it was decided at the time to perform an exfoliative cytological examination instead of an incisional biopsy. Cytologic smears showed epithelial cells and multiple round structures compatible with P. brasiliensis in Papanicolaou staining. Therefore, exfoliative cytology of the oral cavity proves to be an accessible test as a complementary method for diagnosing paracoccidioidomycosis oral lesions.
