Resumo: A 67-year-old man was referred for evaluation of oral candidiasis, reporting burning and pain in the palate and throat for the past 90 days; he reported smoking for over 30 years (20 cigarettes-day), alcoholism, recent unexplained weight loss, and lack of appetite. The intraoral examination showed diffuse, undefined erythematous areas with extending from the soft palate to the oropharynx. The main hypothesis was erythroplasia or squamous cells carcinoma. An incisional biopsy was performed at the intersection of the hard and soft palate. Histopathological examination showed a lining epithelium exhibiting bulbous epithelial crests, loss of stratification and intercellular cohesion, hyperchromatic nuclei, cellular and nuclear pleomorphism, typical and atypical mitoses, nucleoli evident in all epithelial layers, and a lamina propria with a dense predominantly lymphocytic inflammatory infiltrate, compatible with carcinoma in situ. The patient was referred for head and neck surgery to treatment and he will be follow-up in our service.
