Resumo: A 47-year-old man, rural worker, presented with "bubbles on lips" of undetermined time. During the anamnesis, complained weight loss, local symptoms, denied allergies or medication use, reported dysphagia, smoked tobacco (20 cigarettes per day), and alcohol daily use. On extra-oral examination, it was observed lip edema and right submandibular lymphatic chain. The oral clinical examination showed multiple ulcerated lesions with hardened raised edges, moriform aspect, in mucosa and semi-mucosa lower lips were diffusely and extensively, exhibiting macrocheilia and lip stiffening, symptomatic. The diagnosis hypothesis was Paracoccidioidomycosis and Squamous cell carcinoma. Incisional biopsy was performed on the lower lips, and a chest X-ray was requested, which showed alteration. The histopathological examination confirmed the diagnosis of Paracoccidioidomycosis. The patient was referred for treatment with a doctor and follow-up in our service.
