Resumo: A 31-year-old female patient diagnosed with invasive breast cancer evolved with a painful gingival lesion during an episode of febrile neutropenia after the first adjuvant chemotherapy cycle. The intraoral evaluation showed a symmetrical, bilateral, and superficial ulcer covered by a pseudomembrane in the lower attached gingiva. The culture analysis was performed revealing infection by Pseudomonas aeruginosa. The treatment included antibiotic therapy and conservative wound debridement with complete tissue repair after 2 weeks. Oncological patients are susceptible to several opportunistic pathogen infections, especially during nadir. In this sense, clinicians should be aware of unusual necrotizing oral infection presentations, as shown in this case, which demands early diagnosis and a correct clinical approach.
