Resumo: Condyloma acuminatum occurs more often in teenagers and young adults, being rare in elderly patients. A 81-year-old woman was referred to our service for evaluation of a lesion on the left side of the tongue. According to the patient the lesion appeared three years ago and increased in size over time. Her medical history was significant for hypertension, cardiac arrhythmia, arthrosis, osteoporosis, and gastritis. She was a non-smoker and non-alcoholic. Intraoral examination revealed an extensive nodular lesion of sessile base with multiple papillary projections showing white, pink and reddish areas. The diagnostic hypotheses included condyloma acuminatum, papilloma, verrucous carcinoma and squamous cell carcinoma. A biopsy was performed and microscopic evaluation showed parakeratinized epithelium with acanthosis, blunt projections and koilocytosis, favoring the diagnosis of oral condyloma acuminatum. The patient was treated by surgical excision under local anesthesia. One-year follow-up revealed no recurrence of the lesion.
